Billing and Insurance

Who do I contact if I have a billing question?

For billing questions, you may contact Brenda Jones at (205) 345-1520 ext. 102 Monday through Thursday from 8:00am-3:00pm. Or, you may contact Kristi Moore at (205) 345-1520 ext. 105 Tuesday through Friday from 8:00am-3:00pm

Do you accept my insurance?

We are in-network providers for: Alabama BlueCross BlueShield, Blue Advantage, Humana-Military, Medicare, Medicaid, Phifer Wire, and Tricare-Military.

Office Visits

What do I need to bring to my office visit?

All patients should bring their insurance card, drivers license or some other form of photo identification, and a credit or debit card. Printing and completing Patient Forms and bringing them with you to your appointment will make the check in process quicker and easier. Before your first visit and once a year if you are an established patient, a Patient Information Form should be completed. All patients should review, print and sign a Patient Financial Policy and a Credit Card on File Authorization form. Once a year, all patients should complete a Review of Systems form. All patient forms can be downloaded from here or just by clicking on the name of the form above. All new patients should also read our Notice of Privacy Practices (HIPPA) and print and sign Patient Consent for Use and Disclosure of Protected Health Information (HIPPA) and a Receipt of Notice of Privacy Practices form. First time patients coming for laser treatment will also need to complete a laser technology form. What if I need to cancel an appointment?

Please call (205) 345-1520 as soon as possible. After office hours, you can call the same number or email your cancellation to [email protected]. If you fail to keep your appointment and you do not cancel at least 24 hours prior to that appointment, you may be subject to a $50.00 to $100.00 fine. You will be unable to reschedule a visit until after the fine is paid.

How do I get an appointment?

Currently, we are not accepting new patients. Existing patients may call (205) 345-1520 during regular office hours. A staff member will help you schedule an appointment. You may also request an appointment through the home page of this website. One of our schedulers will then contact you by phone or email to make the arrangements. We will make every effort to accommodate your needs. However, our providers are very busy and it may be difficult to give you the exact date and time requested. Patient Information

I had lab work or a biopsy done. How do I find out the results?

Typically, you will be notified of your laboratory or biopsy results within a week. If you have not heard from someone in our office within two weeks after your visit, please call and ask for your results. During normal business hours, call (205) 345-1520 and ask to speak to the nurse of the doctor that ordered your lab work.
